Opt Out Form Login

If you are a Class Member and you want to “opt-out” or ask to be excluded from this lawsuit and the Settlement, you must complete an Exclusion Request. If your Exclusion Request is properly completed and timely received, you will receive no benefits or payment from the Settlement, but you will retain your right to sue on your own.

To be effective and to exclude you from this lawsuit and the proposed Settlement, an Exclusion Request must be completed and submitted no later than August 8, 2026. All submitted Request for Exclusion Forms are subject to review and verification.

To start your online Exclusion Request, you must login with your Unique ID and PIN. You can find your Unique ID and PIN on the Notice you received.

You may also mail in your Request for Exclusion by following the directions in Question 15 and sending your letter to:
